Certificates and private letters for third party organisations and schools are not covered under NHS funding, therefore a fee will be applicable for the completion of such documents.
We can only do these for our registered patients.
In the first instance, you will need to bring your form into the medical centre so the receptionist/secretary can you advise you of the correct fee – which has to be paid in advance.
Due to the volume of requests for private certificates and private medical reports, they can take approximately 2-3 weeks for completion from the date of payment.

Why does it sometimes take my GP a long time to complete my form?
Time spent completing forms and preparing reports takes the GP away from the medical care of his or her patients. Most GPs have a very heavy workload.We always ensure the clinical needs of patients takes priority.
I only need the doctor’s signature – what is the problem?
When a doctor signs a certificate or completes a report,it is a condition of remaining on the Medical Register that they only sign what they know to be true. In order to complete even the simplest of forms, therefore, the doctor might have to check the patient’s entire medical record. Carelessness or an inaccurate report can have serious consequences for the doctor with the General Medical Council (the doctors’ regulatory body) or even the police.
Why do I need a Medical Examination?
Some forms require the doctor to state that you are fit to perform certain activities. Without assessing you at the time it is not always possible to confirm this. Some forms will state that a medical examination is not necessary and that information from your medical records is sufficient– check with whoever is requesting the form if in doubt.
