Maternity Self Referral form

Step 1: Make an appointment to see your GP to confirm your pregnancy 

Step 2: Then, if you would like to have your baby at the Norfolk and Norwich University Hospital, please complete the self-referral form www.nnuh.nhs.uk/maternity-self-referral-form

Step 3: Once you have done step 1 and 2, call Medicom on 01603 481222 to make your first midwife appointment. Your first appointment should be when you are between 8 and 11 weeks pregnant.
